Video/DVD List & Loan Agreement Form

Members of HWS may borrow instructional videos from the Society’s collection under the following terms and conditions:

• Contact the Librarian (Jane Houk). Fill out the Video Loan Agreement form. A $5 refundable deposit applies per video/DVD. Loan period is a maximum of one month for a maximum of 3 videos/DVDs at a time.
• The Video Loan Agreement form may be copied and a copy retained by the borrower; the original is held by the Librarian.  A blank form is included in the monthly newsletter.
• Videos or DVDs may be borrowed and returned to the Librarian at the monthly HWS board meetings. Other arrangements for pickup and drop off may be made by mutual agreement.
• If the borrowed tape/DVD is lost, damaged, stolen or destroyed, borrower must pay the replacement cost to HWS.
• Videos/DVDs may not be reproduced or lent to a third party.
• As new programs are acquired and made available to borrow, they will be listed in the newsletter. All tapes are VHS unless otherwise indicated. We are gradually converting our library from VHS to DVD, so please make sure the program you request is in the format you can play.
